Introduction

CATIE is hosting its national, bilingual forum November 23 and 24, 2017. Our theme of Transforming our practice: New knowledge, new strategies is in response to a pressing need for dialogue and learning regarding the frontline implications of new knowledge and approaches in HIV and hepatitis C prevention, testing, treatment, care and support.

This event will bring together 400 people on the front lines of the Canadian response to HIV and hepatitis C, including people living with HIV, people with lived experience of hepatitis C, community workers, healthcare providers, public health practitioners, researchers,  program planners  and policy-makers. The CATIE Forum will be an opportunity to learn more about advances in HIV and HCV prevention and treatment science and programming knowledge, discuss the implications of this frontline work, share promising practices, and strategize about building and maintaining effective programs.

The CATIE Forum will also provide an opportunity to network, learn from one another, and develop new collaborations.

Registration

Each individual planning to attend the CATIE Forum 2017, Transforming our practice: New knowledge, new strategies must complete a separate registration form.

Registration Categories and Fees
Early-bird Registration until Friday, October 13, 2017 $199
Regular Registration Saturday, October 14, 2017 and later $299
Unemployed and Community Members*   $ 0

Registration fees include all sessions, continental breakfast, lunch and nutrition breaks on both days and a Welcome Reception.

*The Unemployed and Community Members rate is offered to people living with HIV or hepatitis C who are unemployed, and to volunteers in the HIV or hepatitis C sector who are not employed in the sector.  To request a free registration please complete an application for financial assistance.

Methods of Payment

Credit Card and Debit Card

Credit card and debit card payments are accepted through Stripe at the time of registration.

Cheque

If you are paying by cheque include CATIE 2017 and the name of the registrant(s) on the cheque and make the cheque payable to CATIE. Send your payment to the contact at the bottom of this page. Funds are due in Canadian dollars and must be drawn on a Canadian bank. Once payment has been received, a receipt will be sent to the email address provided during the registration process. Cheque payments must be received by Friday, October 13, 2017. After this date, registrations that have not been paid in full will be automatically cancelled.

Cancellation & Refund Policy 

Cancellations or substitutions must be received in writing either by email at jvanveen@catie.ca or fax at 416-203-8284.

Refunds will be available until Friday, October 13, 2017and will be subject to a cancellation fee of $25 per person for all registration categories, except the Unemployed and Community Members category.

As of Saturday, October 14, 2017 no refunds will be provided, however substitutions or transfers of registration to another person can be made until Friday, November 10, 2017.

CATIE is funded by the Public Health Agency of Canada to provide resources and educational opportunities to service providers and people living with HIV and/or HCV in Canada. CATIE is unable to provide financial assistance or letters of invitation to attend the conference to international registrants.

Financial Assistance (Registration only)

CATIE has committed to offering free registration to people living with HIV or hepatitis C who are unemployed, and to volunteers in the HIV or hepatitis C sector who are not employed in the sector who can have the greatest impact back in their communities.

Applications for the The Unemployed and Community Members rate to cover the cost of registration only will be accepted until Friday, September 8, 2017 at 4:00 p.m.
